to Transition from Pediatric to Adult Medical Care

PROGRAM DESCRIPTION:
Stepping from childhood into adulthood is never easy, but it’s even more difficult for the child with special health care needs. This lecture will focus on what is involved in switching from pediatric care to using adult specialists, a process that is gradual and works better with planning. Dr. Cynthia Peacock, whose clinical practice includes patients with diagnoses such as spina bifida, sickle cell disease, congenital heart disease, autism, down syndrome, cerebral palsy, and survivors of childhood cancer, will talk about how to think about that transition. Since there are few physicians - and even fewer clinics - specializing in Transition Medicine, Dr. Peacock will talk about how to create a transition team of your own, how to work with your pediatric specialists, what to look for in specialists for the young adult, and what resources are available for patients in transition. She will also be available to answer questions about specific challenges facing participants.
About the Speaker: Dr. Cynthia Peacock is the Medical Director of the Baylor Transition Medicine Clinic, a practice that focuses on helping young people with special health care needs to transition services from the pediatric-subspecialist to the adult-subspecialist. Dr. Peacock entered Baylor College of Medicine in 1991 after 15 years of a successful career in nursing. She completed the Medicine-Pediatrics Residency Program at Baylor College of Medicine in 1999. After graduation, she worked two years as a flight physician for Texas Children's Hospital Air Medical Transport Service. During this time she continued her affiliation with the Med-Peds Residency Program through supervising residents in the continuity clinic and assisting with recruitment. In 2001, she joined the Medicine-Pediatric Faculty at Baylor College of Medicine full-time.
